Credentialing Checklist

A practical credentialing checklist for tracking the steps that most often delay provider readiness.

Use this checklist to organize payer enrollment, supporting documents, follow-up steps, and readiness questions before the delay becomes a revenue problem.

Use This Checklist When

Provider setup is moving, but readiness still feels hard to track.

  • A provider has been hired and the payer-enrollment picture is scattered
  • Leadership cannot clearly tell which applications are complete, pending, or stalled
  • Documents, portals, and follow-up steps are living in too many places
Checklist

Review these items before assuming readiness is on track.

  • Confirm CAQH profile is current and attested
  • Confirm PECOS and NPI details are complete where relevant
  • List each payer and the status of application, follow-up, and missing items
  • Identify who owns the next action for each payer or enrollment step
  • Confirm provider documents, signatures, and licensure details are easy to locate
  • Document the expected billable date and what is still blocking it
What This Helps Surface

The checklist is meant to expose where enrollment work is active but visibility is weak.

Most readiness delays do not come from one missing form alone. They come from tracking gaps, unclear ownership, and follow-up steps that are not being carried in a way leadership can actually see.
