Claims & Reimbursement

Medical Billing Services For Physicians & Outpatient Practices

7% of collections for a solo provider, with a $300 monthly minimum and no setup fee. The rate drops as you add providers, down to 4.5%.

Ask us anything, for as long as you're a client. How to bill as a new practice, how to bill a certain insurance company, what a denial means. Send it in your Command Suite or by email, and we reply within one business day, usually the same day. If a question turns into new work, we send a written quote before we start.

We run medical billing for independent physicians and outpatient practices nationwide, from claims and denial follow-up to payment posting and A/R, inside the systems you already have.

Experience Behind The Service

Professional Billing, Outpatient Revenue Cycle, And Owner-Level Operations.

Updated October 2026

Our founder’s background includes professional billing and collections at OHSU, nationwide outpatient practice support at Clinicient, revenue cycle leadership for a multi-provider physiatry group, and finance leadership during a dental group’s expansion from two to six clinics. Behavioral health is still our deepest specialty.

A claim can fail long before it reaches the payer. Nobody checked coverage, the note doesn’t support the code, or the provider isn’t enrolled at that payer yet. Or the payer pays it and the payment posts to the wrong line.

You work directly with people who know how each of those steps affects the next. We work in the systems you already use and put the scope, price, who does what and the limits in writing before we start.

Where It Leaks

Fewer Claims Going Out Wrong.

We fix what causes denials instead of just working them harder.

Eligibility Checks, If You Want Them

We can verify coverage before visits when it is added to your written scope.

Coding Alignment

Your providers choose the codes, and we check each claim against payer rules, which cuts denials.

Clean Claims

Scrubber edits worked down so more claims clear on the first pass.

Denial Root Cause

We track denials by payer, code and provider, so we fix the pattern instead of resubmitting the same claim.

A/R Follow-Through

We work aging by priority, starting with the biggest balances and the claims closest to their timely filing limit.

Revenue Visibility

Know Where Every Claim Stands.

Your denial rate, A/R aging and payer mix sit in one place, so the report leadership reads matches what’s happening. When billing turns up a problem that started upstream, like a plan entered on a patient’s account with no effective date, our revenue cycle management work traces it.

How It Works

From Review To Steady Cycle.

1

Review

We map your denial rate, A/R, payer mix, and where coding, eligibility, or enrollment is costing you.

2

Stabilize

We clean up claim submission and work rejections and denials by root cause. Eligibility verification is added only through a separate written scope.

3

Report

Each month you see what every provider collected and what’s still open in A/R.

Who We Bill For.

Outpatient & Specialty Practices

A mix of office visits and other services, billed to several payers that each have their own rules.

Solo & Small Groups

Clinicians doing their own billing on top of a full patient schedule.

Multi-Provider Groups

Provider-level revenue visibility and credentialing status that move together.

Scaling Practices

Adding providers, locations, or service lines without the billing lagging behind.

What A Clean Claim Requires

A clean claim is the result of four things going right before it leaves your office.

Eligibility Before The Visit

Coverage, plan, and any carve-out should be verified before the appointment, by your front desk or by us when it’s in your written scope. Checking after the visit turns a routine correction into a write-off risk.

Charge Entry Against The Fee Schedule

Charges are entered against your contracted rates so underpayments are visible as underpayments instead of disappearing into the adjustment column.

Remittance Posted To The Right Line

Payments and adjustments post to the service line they belong to. Posting at the claim level hides which code is being paid short.

Aging Worked By Cause

Old claims are grouped by why they're old, not by how old they are.

FAQ

Frequently Asked Questions

Which Specialties Do You Bill For?

Psychiatry, PMHNP and therapy practices are our deepest work. Our founder has also run revenue cycle work for a physiatry group, worked with outpatient rehab practices around the country and worked claims in professional billing at OHSU. Claims, denials and A/R follow the same steps in every specialty, and if yours needs coding expertise we don’t have, we tell you before we start.

Do You Replace Our EHR Or Billing System?

No. We work inside the systems you already use. When the setup is what’s causing denials, we fix the setup, and nobody has to migrate.

How Is Billing Priced?

By practice size. Ongoing Billing runs from 7% of collections for a solo provider down to 4.5% at 26 to 50 providers, with a $300 monthly minimum for a solo provider, and no setup fee. Eligibility checks and prior authorization need a separate written scope. The included work, any add-ons, the initial term and the cancellation notice are all in the written quote before you sign.

What Drives Most Denials?

Most start before anyone builds the claim. Nobody checked eligibility, the service needed an authorization nobody got, or the code doesn’t match what the note documents. We fix the cause so the next claim doesn’t fail the same way, instead of only resubmitting this one.

Do You Bill Across Multiple Payers?

Yes. We scrub, submit and follow up on claims for commercial plans, Medicare and Medicaid, and each payer’s rules go into your billing setup, from timely filing limits to the modifiers it expects. Denials get tracked by payer, code and provider, so a pattern at one payer shows up as one cause to fix.

Not on the panels yet? Start with what physician credentialing costs.

What Would Fixing It Cost?

Ongoing Billing, Automation Programs and the Operations Partnership each have a published price and a written scope. Tell us what keeps going wrong, and we’ll point you to the right one.
