Maine Behavioral Health Credentialing and Payer Enrollment
MaineCare never went to managed care. You enroll once through the Health PAS Online Portal and there’s no plan roster behind it, no MCO contracts, nothing to chase. A 2025 bill to bring in three plans, LD 1878, was voted Ought Not to Pass on May 21, 2025 and died. What Maine has instead is a section problem. MaineCare Section 65 doesn’t list prescribers as independent practitioners, so a solo psych NP enrolls under Section 14. Get that one wrong and you’re applying at a door you can’t walk through.
Less to chase here, so the wrong door costs you more.
Maine takes work off the front end and puts it into getting the details right. There’s less to chase here than almost anywhere, so the cost of aiming at the wrong door is higher.
Fee-for-service, and nothing behind it
- MaineCare is fee-for-service statewide. One enrollment reaches every member. There is no plan-by-plan contracting step behind it
- LD 1878, which would have directed the department to contract with three MCOs from 2029, was voted Ought Not to Pass on May 21, 2025 and died under Joint Rule 310 on May 27. Watch it, don’t plan around it
- Acentra Health, the organisation people still call Kepro, runs prior authorization, continued stay review and referral management through the Atrezzo portal. That’s authorization plumbing, not a second network
- You don’t credential with Acentra. You register for Atrezzo once you’re a MaineCare provider
Section 14 or Section 65 decides everything
- Section 65.02-1 defines Independent Practitioner as a Licensed Psychologist, Psychological Examiner, LCPC, LCSW or LMFT. Prescribers are not on that list
- Everything else in Section 65 routes through Mental Health Agencies and Substance Use Agencies licensed under 10-144 C.M.R. ch. 123
- A PMHNP appears in Section 65 as a Clinician, under APRN-PMH-NP and APRN-PMH-CNS, which is how a prescriber delivers those services inside a licensed agency
- On your own the door is Section 14, Advanced Practice Registered Nursing Services. Section 14.06 reimburses an independent practitioner at the published Section 90 physician-services amounts
Thirty days, one list, sixty to decide
- Within 30 days of first receiving your application the carrier has to review the whole thing and, if it’s incomplete, return it with one comprehensive list of everything needed
- Then 60 days to decide a completed application, extendable only by written notice inside those 60 days, with a 180-day outer limit no matter what
- A carrier that can’t decide inside 60 days has to notify the Bureau of Insurance in writing before the deadline passes, and file a remediation plan where the delay isn’t specific to one application
- The single-list rule matters as much as the deadline, because it ends the drip of new requests that keeps a file open for months
Paid back to the day your file was complete
- If the carrier credentials you, it has to pay claims for services you rendered before credentials were granted, back to the date your complete application was submitted
- That makes a genuinely complete first submission worth money, and it’s why we don’t send a partial file to look fast
- Separately, a carrier may not require a provider to have a home address within the State before accepting an application, which removes an obstacle several other states leave in place for telehealth practices
- The gate that does apply is a licence where the patient is sitting. Community Health Options puts that on its own network form
Getting on a panel isn’t the same as getting paid.
Most of the waiting happens in the gaps between these five steps. We work all of them, and we tell you which one you’re actually sitting in.
MaineCare is one application. Commercial is a short list.
Because there are no plans behind MaineCare, it’s often worth sequencing early. It’s one application, not one plus five.
Trading Partner Account first, then the application.
Enrollment is the online Provider Enrollment Application in MIHMS, the Maine Integrated Health Management Solution, submitted through the Health PAS Online Portal. Gainwell Technologies runs enrollment for the state. Step one is a Trading Partner Account on the portal, and without it you can’t submit the application or check claims later. An executed MaineCare Provider Agreement is required before you can participate, and a new agreement is needed for a new enrollment, a re-enrollment or a reactivation. Address changes, added service locations and name changes take an updated agreement instead.
Anthem is first, and its behavioral door is Carelon.
Anthem holds the Blue licence for Maine and runs the largest network in the state, covering State of Maine employees, the Maine Education Association Benefits Trust and the Maine Municipal Employees Health Trust alongside its own book. After that the list is genuinely short: Harvard Pilgrim, Community Health Options, UnitedHealthcare through Optum, Martin’s Point on the Medicare and TRICARE side, and Aetna and Cigna as employer and national account business.
Who you’ll apply to, and how long it usually takes.
These are typical ranges from a complete application, with the 60-day decision requirement in 24-A M.R.S. section 4303 sitting underneath them. Payers control their own timelines, so we track them rather than promise them.
Anthem Blue Cross and Blue Shield in Maine
- Usually: about 90 to 120 days from a complete application, with the statutory 60-day ceiling on a decision once the file is complete and a documented extension route out to 180 days
- Behavioral health: the door is Carelon Behavioral Health, not the medical network, for commercial and Medicare Advantage alike. Credentialing and recredentialing run through the Carelon National Provider Service Line, with a Maine network integration mailbox. Claims and eligibility run through Availity
- Bonus coverage: this contract is also how you reach State of Maine employees, the Maine Education Association Benefits Trust and the Maine Municipal Employees Health Trust
- Watch: set CAQH access to Global, or Carelon can’t see your updates and demographic changes have to go through both systems
Separate door: Carelon Behavioral Health
Harvard Pilgrim Health Care
- Usually: Point32Health says credentialing and contracting generally run about one to two months from a complete application, longer if anything is missing. Credentialing runs through the HCAS process using CAQH
- Behavioral health: in house since the November 2023 insourcing, completed January 1, 2025. Your old Optum route doesn’t reach these members
- 2026 network changes: Maine Choice expanded statewide, and Northern Light Health hospitals sit in the Preferred tier of Maine Choice Plus across Aroostook, Hancock, Piscataquis, Penobscot and Somerset counties
- Watch: a National Access product launched January 1, 2026 using the Harvard Pilgrim network in its five-state region and UnitedHealthcare’s Choice Plus network elsewhere, with no out-of-network benefit
Community Health Options
- Usually: contract first, then credentialing. Health Options requires a signed participation agreement, or an agreement with an already-contracted entity, before credentialing starts, and you can’t see members until its Credentialing Committee approves you. Its own guidance describes at least 30 days for data entry then up to about 60 business days to finish
- How it goes in: the Provider Network Request Form to the contracting mailbox, with adds and changes going to the data integrity mailbox. Verisys is the credentialing verification organization and works from the contact listed in CAQH, so that contact needs to be a real person
- Why it matters: one of the last ACA co-ops still operating and a genuine network here, though its individual-market share slipped about five points during the 2026 open enrollment
- Watch: the network form asks whether services are telehealth-only and notes you must be licensed in the state the member is located in
UnitedHealthcare and Optum
- Usually: 60 to 120 days from a complete application, started through Join Our Network inside Provider Express. CAQH is required for Maine credentialing per Optum’s own Maine page, so get the CAQH ID first
- Why it’s near the front: Optum accepts MDs, DOs, PAs and RNs with prescriptive authority in all counties, and openly recruits telemental health, medication assisted treatment prescribers, Express Access providers who can see a member within five business days, and EAP providers
- Rare detail: it names Androscoggin, Kennebec, Lincoln and Sagadahoc counties as priority areas for some provider types. Payers almost never tell you where they want you
- Watch: this is a separate application from core UnitedHealthcare medical
Separate door: Optum Behavioral Health
Martin’s Point Health Care
- Usually: no published turnaround. Martin’s Point credentials through its own Credentials Committee, so confirm with its network management team at submission
- What it is: not a commercial group carrier. Its lines are Generations Advantage, one of Maine’s larger Medicare Advantage plans, and the US Family Health Plan, which is TRICARE Prime for military families and retirees in northern New England
- Why it matters: it credentials nurse practitioners and other prescribers directly rather than through a behavioral vendor, and if your panel skews Medicare-age or military there’s no other route to these members
- Watch: it was reported as discontinuing at least one plan for 2026, so confirm the current product list before you build around it
Aetna
- Usually: a request for participation, an answer on network need within about 45 days, then credentialing pulled from CAQH. Commonly 60 to 120 days end to end
- Path: behavioral health professionals use Aetna’s separate behavioral health request for participation, not the medical one
- Set expectations: Aetna isn’t among the carriers selling individual or small group coverage in Maine for 2026, and reporting has it scaling back Medicare Advantage here for 2026, affecting roughly 1,400 members while keeping plans in some southern and central counties. What’s left is mostly large group and national account business
Cigna Healthcare and Evernorth
- Right now: Evernorth paused new individual and clinic behavioral health applications on June 1, 2026 and has signalled a reopen after September 1. Facilities are exempt and anything filed before June 1 keeps processing
- Usually: up to about 90 days to join the behavioral network once it’s open
- Set expectations: Cigna isn’t among the carriers offering individual or small group coverage in Maine for 2026 either, so its presence here is employer and national account business. We sequence the other panels first and queue this one
Separate door: Evernorth Behavioral Health
Don’t build a Maine launch around Mending, formerly Taro Health. It notified the Maine Bureau of Insurance on June 8, 2026 that it will stop offering health insurance in Maine as of January 1, 2027. Roughly 1,100 members keep coverage through the end of their plan year and shop for 2027 in the open enrollment that starts November 1, 2026, and small groups run to their renewal dates. Mending was one of four carriers on CoverME.gov for 2026 and held about two percent of the individual market after expanding into six new counties, so you may well have current Mending patients. The practical point is sequencing: those patients need a different in-network route for 2027, which usually means getting the Anthem, Harvard Pilgrim or Community Health Options contract in place first. Humana is a Medicare Advantage conversation in Maine, not a commercial one. Medicare, Medicare Advantage and MaineCare work is scoped and priced separately from a commercial package.

Maine has several large blocks of covered lives that don’t look like carriers. The State of Maine employee health plan runs on the Anthem network. The Maine Education Association Benefits Trust holds a group contract with Anthem covering roughly 69,000 people, with behavioral health through Carelon and an Anthem-run employee assistance programme behind it. The Maine Municipal Employees Health Trust covers municipal employees across the state on the same network. In all three the Anthem contract is the route, not a separate trust contract, which is one more reason Anthem usually goes first. The US Family Health Plan is TRICARE Prime for military families and retirees in northern New England, and you reach those members through Martin’s Point credentialing rather than a national TRICARE contract, so it’s easy to miss if you only think in big-carrier terms. You name the plan. We find the real route.

In Maine that answer decides whether you enroll under Section 14 or need an agency route, and whether the practice entity itself needs a DHHS licence. Better settled in intake than mid-application.
- Your individual NPIPlus the Type 2 if you bill under a group or entity. MaineCare ties the Provider Agreement and every service location to these, so we want them right before the application goes in.
- Active Maine licence and DEAOptum’s Maine page is explicit that it takes RNs with prescriptive authority, and Community Health Options’ form says you must be licensed in the state the member is located in. That’s the gate for telehealth reach too.
- CAQH attested and authorizedYou attest it. We keep it current. Anthem, Harvard Pilgrim, Community Health Options, Optum, Aetna, Evernorth and Martin’s Point all pull from it, and Maine’s 60-day clock only starts once the file is complete. For Anthem, set access to Global so Carelon can see your updates.
- Delegated access to the portalSo we can set up the Health PAS Trading Partner Account and complete the MaineCare enrollment on your behalf, with no password sharing. We also need the mailing address and contact you want on file, because that’s where the revalidation notice goes five years from now.
- Malpractice certificateThe current declarations page naming you.
- Entity name, EIN and W-9One per tax ID you bill under.
Depending on what you plan to bill, Maine may want the practice itself licensed, separately from your own licence. The state’s term is a Behavioral Health Organization licence from the DHHS Division of Licensing and Certification under 10-144 C.M.R. ch. 123, and Maine licenses mental health programmes and substance use disorder treatment programmes separately, so a practice offering both would be looking at two applications and two licences. Two places make it concrete. MaineCare Section 65 routes agency-style behavioral health through Mental Health Agencies and Substance Use Agencies holding that licence. And a June 6, 2025 DHHS bulletin states that Section 65 intensive outpatient providers must be licensed under ch. 123, that mental health programmes need medication management on the licence and substance use programmes need medication assisted treatment on it, with 30 calendar days to add it. A solo prescriber doing outpatient medication management under Section 14 usually sits outside all of this, but the line follows the services billed rather than the sign on the door.
