Louisiana Behavioral Health Credentialing And Payer Enrollment

If you’re a behavioral health provider getting credentialed in Louisiana, ask one question first: do you hold hospital privileges or sit on the medical staff of a rural health clinic or FQHC? Under La. R.S. 46:460.61(F) that satisfies Medicaid managed care credentialing, though you enroll in the state portal and need each plan’s contract. Healthy Louisiana is five statewide plans, commercial runs on CAQH with a 90-day clock on carriers, and Louisiana Blue never backdates your effective date. Here’s the order, the shortcut, and what only you can sign.

Why Louisiana Is Different

The Shortcut Nobody Asks About, And The Two Clocks Behind It.

Louisiana wrote more of this into law than most states did. The law only turns into money if somebody asks the right question early and keeps the dates. That’s the part we do.

The Act 143 Shortcut

La. R.S. 46:460.61(F)
  • Hospital privileges, or medical staff at a licensed rural health clinic or an FQHC, is treated as satisfying Medicaid managed care credentialing
  • Added by Act 143 of 2022, and the plans have a defined way to apply it. AmeriHealth Caritas Louisiana asks for an email with Add ACT 143 in the subject line plus its provider enrollment form
  • It does not skip your state portal enrollment, and it isn’t the contract
  • Easy to miss, because nobody asks you about hospital privileges when you’re building an outpatient practice

Sixty Days, And The Remedy Runs Itself

Medicaid managed care, La. R.S. 46:460.61
  • 60 days for a plan to finish credentialing, with a 30-day notice on defects and a 45-day notice if a verification hasn’t come back
  • Miss it, and if the plan hasn’t loaded you into both its claims system and its electronic provider directory or denied you, you’re deemed credentialed and paid at the Medicaid fee schedule
  • No demand letter and no regulator. It’s self-executing, which is rare
  • The plan can recover the in-network and out-of-network difference if it later denies you, but only if it noticed you and started within 30 days of that determination

Ninety Days, And Your Second Office Comes Free

Commercial, La. R.S. 22:1009
  • 90 days to finish credentialing once the carrier has everything, a 30-day notice on known defects, and a 60-day notice on outstanding verifications
  • Reaches HMOs, preferred provider organizations, dental benefit plans and the state’s Office of Group Benefits programs
  • (B)(5) and (6): credentialed at one Louisiana location means credentialed at any other Louisiana location on written notice, effective immediately, with no other requirements allowed
  • Portability has one condition. Another currently credentialed provider has to have legally practiced there at least 30 days earlier

Louisiana Blue Does Not Backdate

The counterweight, and it’s a big one
  • Your effective date is the date on the signature page of the provider agreement, not the credentialing approval
  • You’re out of network until the Welcome to the Network letter lands, and earlier claims process out of network, which can send payment to the patient
  • Louisiana has no equivalent of the retroactive-reimbursement rules a few other states wrote. The 90-day clock is a deadline on the carrier, not a safety net for your first month
  • So we sequence Louisiana Blue first and file complete, rather than filing fast and fixing later

Where The Delays Happen

Getting On A Panel Isn’t The Same As Getting Paid.

The waiting happens between these five steps more than inside them. We work each gap and tell you where your file is sitting.

Credentialed›contracted›loaded›connected›payable

Medicaid And Commercial Run On Different Clocks Here.

Two Tracks

Sixty days on one side, ninety on the other, and only one of them has a remedy behind it. Most Louisiana practices want both, and they can run in parallel.

Healthy Louisiana

Five Plans, All Statewide, And No Behavioral Health Vendor For Adults.

Aetna Better Health of Louisiana, AmeriHealth Caritas Louisiana, Healthy Blue, Humana Healthy Horizons and Louisiana Healthcare Connections cover the whole state, so there’s no parish map to sort out. Specialized behavioral health folded into these same plans on December 1, 2015, so for an adult or a routine outpatient case the plan is the payer and there’s no separate vendor to chase. One carve-out is left: the Coordinated System of Care for children and youth at risk of out-of-home placement, run by Magellan Health of Louisiana under its own contract. Healthy Blue leaves when its contract ends on December 31, 2026, so from January 1, 2027 the list drops to four.

How you enrollThrough the Louisiana Medicaid Provider Enrollment Portal at lamedicaid.com, run by the state’s fiscal intermediary Gainwell Technologies. Even a provider who only ever plans to see managed care patients has to enroll and be screened by the state. Louisiana used to let you skip that and just contract with a plan. The portal ended it in 2021.
The 120-day letterIf a plan credentialed you before you enrolled with the state, Gainwell sends an enrollment invitation letter to whatever mailing address that plan has on file for you, and you have 120 days from the letter to finish. Miss it and claims deny and your provider number deactivates. Those rebaselines run roughly every two months. The Provider Portal Enrollment Lookup Tool shows where you stand and updates daily.

Prescribers, read thisSince July 1, 2023 a pharmacy point-of-sale claim denies when the prescriber isn’t enrolled with Louisiana Medicaid. A commercial-only practice can still have a patient turned away at the counter. Separately, a Louisiana provider number with no billing activity for 18 months is subject to autoclosure, which catches people who enrolled early and hadn’t started seeing patients yet.
Two cycles, not oneThe state screens and revalidates you at least every five years. Each plan recredentials you every three years on its own schedule, and the Department of Health is direct that a provider who doesn’t complete a plan’s recredentialing gets disenrolled from that plan.

Commercial

Louisiana Blue Does Most Of The Work, And Its BH Door Moved.

The American Medical Association’s competition study, on 2024 data, puts Louisiana fifth least competitive in the country for commercial coverage, and Louisiana Blue is the largest carrier by a wide margin. It carries the commercial group book, the individual market and the Office of Group Benefits state employee plans. Vantage Health Plan, the Monroe independent people still ask about, exited group health on January 1, 2024 and became part of Louisiana Blue.

Your CAQH stays yoursYou complete and attest your own profile. We’re added as an authorized practice manager, so we keep the practice data current during your project and get each payer authorized. After that, Credentialing Maintenance keeps it current. If you want us to maintain your profile, you share access securely and change the password when we’re done. Both Louisiana credentialing statutes name either the state form or the CAQH format, and the large payers here run on CAQH, so an unattested profile blocks several applications at once.
Behavioral health has its own doorUnitedHealthcare goes through Optum and Provider Express. Cigna goes through Evernorth. Aetna keeps it in house, on the same request for participation as medical. Louisiana Blue took behavioral health authorization and case management in house on January 1, 2026, and Lucet no longer runs it, so any instruction pointing you at Lucet is out of date.
The state form, and why CAQH still winsLouisiana publishes a Louisiana Standardized Credentialing Application Form through the Department of Insurance, and both statutes say a payer shall accept either that or the current CAQH format, at the payer’s discretion. In practice the large payers pick CAQH. We keep both usable so a smaller plan asking for paper doesn’t stop you.
Seeing patients in other statesOne Louisiana Blue contract reaches Blue members who live in other states through BlueCard. Aetna, Cigna with Evernorth, and UnitedHealthcare with Optum are national, so one contract each covers every state you’re licensed in. The gate is a license where the patient is sitting. Medicaid is the genuine exception, and joining another state’s network is its own question, because a few states want a physical presence first.

Who You’ll Apply To, And How Long It Usually Takes.

Louisiana Commercial Payers

These are typical ranges from a complete application, with the 90-day rule in La. R.S. 22:1009 sitting underneath them. Payers control their own timelines, so we track them rather than promise them.

Louisiana Blue

Blue Cross and Blue Shield of Louisiana, and the first contract we sequence
  • Usually: up to 90 days after all required information is received, with the credentialing committee approving twice a month. Louisiana Blue asks you not to call for status until those 90 days have passed
  • How it goes in: a Professional Initial Credentialing Packet through DocuSign, bundling the CAQH application, the Attachment A location hours form, the iLinkBlue service agreement, a business associate addendum, EFT enrollment and an administrative representative registration. There’s a separate packet for joining an existing group
  • Behavioral health: handled in house since January 1, 2026, with medical-necessity appeals going directly to Louisiana Blue. Lucet is out
  • Watch: no backdating, ever. And recredentialing every three years goes to the single correspondence address on file, so a stale address is how a good contract dies

UnitedHealthcare And Optum

The strongest published opening in the state
  • Usually: 60 to 120 days from a complete application
  • Path: behavioral health starts on Optum’s Louisiana page, where you attest to the participation criteria, then goes through Join Our Network in Provider Express. That’s a separate application from core UnitedHealthcare medical
  • Why it’s first or second: Optum’s own Louisiana page recognizes the state as any willing provider and lists prescribers as actively sought in all parishes, along with telemental health and medication assisted treatment prescribers
  • Watch: UnitedHealthcare left Healthy Louisiana on March 31, 2026, so Medicaid isn’t part of this conversation any more. On the Medicare side it went the other way, and Peoples Health is now a UnitedHealthcare company

Separate door: Optum Behavioral Health

Aetna

Behavioral health in house, on the same form as medical
  • Usually: a request for participation first, then an answer on network need and eligibility, then credentialing pulled from CAQH, then contracting. Commonly 60 to 120 days from start to finish
  • Path: behavioral health professionals use the same Aetna request for participation as medical professionals
  • Watch: keep commercial Aetna mentally separate from Aetna Better Health of Louisiana, which is Healthy Louisiana Medicaid and a different contract

Cigna Healthcare And Evernorth

Behavioral health goes through Evernorth
  • Current application route: Evernorth directs individual providers to its provider portal and publishes separate applications for clinics and facilities. We confirm the right provider category and network requirements before we submit.
  • Usually: Evernorth says joining its network can take up to 90 calendar days, and asks you to re-attest CAQH every 120 days
  • What we do: we include it in your opening set. Evernorth was accepting individual applications through its provider portal when we checked on October 1, 2026

Separate door: Evernorth Behavioral Health

Ambetter From Louisiana Healthcare Connections

A cheap add to a relationship you may already be building
  • Usually: no published standard turnaround. Interest goes in through an online Join Our Network form and the Centene Louisiana network team responds. Plan on a normal commercial cycle
  • Why it matters: Centene runs both the Healthy Louisiana Medicaid plan and the Ambetter marketplace product here, and being in one doesn’t put you in the other
  • Watch: ask for it by name, or it gets left out of a Centene conversation that everyone assumes covered it

Two names Louisiana clients still ask for don’t fit where they think. Humana is a Medicare Advantage and Medicaid conversation in Louisiana, not a commercial group one: it announced its exit from employer-group commercial medical in 2023 and phased out of it, and Louisiana is also one of the ten least competitive Medicare Advantage markets in the country on the AMA’s 2024 data, so that side deserves deliberate scoping. Vantage Health Plan exited group health on January 1, 2024 and became part of Louisiana Blue, with its Medicare Advantage members folded into the Blue Advantage products. And Elevance Health’s proposed acquisition of Blue Cross and Blue Shield of Louisiana was announced in January 2023, and in February 2024 Louisiana Blue paused it again and withdrew its filing with the Department of Insurance. We watch it rather than plan around it. Medicare, Medicare Advantage and Healthy Louisiana Medicaid work is scoped separately from a commercial package.

Carry A Plan We Didn’t List? Add It.

Louisiana has a few that don’t look like insurance from the outside. The Office of Group Benefits is a large block of covered lives for state employees, retirees and dependents, and it’s named directly in the commercial credentialing statute, so the 90-day rule reaches it. Louisiana Blue administers the benefits, and members sit on Preferred Care PPO, Blue Connect or Community Blue depending on the plan and the parish, so you usually reach them through your Louisiana Blue contract and the right network rather than a separate OGB contract. Magellan Health of Louisiana is its own credentialing and contracting relationship for Coordinated System of Care youth. And Gulf Coast building trades, maritime and offshore Taft-Hartley funds are usually reached by joining a rented commercial network rather than by contracting with the fund. You name the plan. We find the real route.

Send These Once And We Can Start.

What We Need From You

  • Whether you hold hospital, RHC or FQHC affiliationPrivileges at a Louisiana licensed hospital, or a spot on the medical staff of a licensed rural health clinic or an FQHC. Under La. R.S. 46:460.61(F) that can satisfy Medicaid managed care credentialing outright. It’s the biggest shortcut in the state and the easiest one to miss.
  • Your individual NPI and taxonomyPlus the Type 2 if you bill under a group or business. The Louisiana portal is built around NPI and taxonomy, and a wrong or missing taxonomy is a common stall. More than one provider type means a separate enrollment for each.
  • Active Louisiana license, DEA and CDSYour license for the provider type, your DEA registration, and your Louisiana controlled dangerous substance license where it applies. Louisiana Blue and Healthy Blue both ask for copies at credentialing and again at recredentialing.
  • CAQH attested and authorizedYou attest it. We keep it current during your project. In Louisiana it’s one of the two formats named in both statutes, and it’s what Louisiana Blue, Healthy Blue, AmeriHealth Caritas Louisiana, Aetna and Evernorth all pull from.
  • Delegated access to the enrollment portalSo we can complete your Louisiana Medicaid Provider Enrollment Portal application and each plan application without anyone sharing a password. We also want the mailing address each plan has on file for you, because that’s where the 120-day invitation letter goes.
  • Every Louisiana location, with hoursLouisiana Blue requires an Attachment A location hours form, wants at least eight hours a week at a listed location for the directory, and caps you at ten locations per tax ID. Your location list also drives the portability provision in La. R.S. 22:1009.
  • Five years of work historyMonth and year, with any gap over six months explained. It lives in CAQH and it’s the single most common reason a Louisiana file sits.
  • Malpractice certificateThe current declarations page naming you, or a carrier-produced roster face sheet.
  • Legal business name, EIN, and W-9One per tax ID. Louisiana Blue’s packet also carries EFT enrollment and an administrative representative registration, so tell us who in your practice should hold portal access.
  • One correspondence address a person readsLouisiana Blue keeps a single correspondence email on file and sends recredentialing there. Gainwell sends Medicaid invitation letters to the address a plan has for you. In this state a stale address is the most common way a good enrollment dies.

Depending on the services you plan to bill, Louisiana may want the practice itself licensed, separately from your own license. The state’s term is a Behavioral Health Services Provider license, issued by the Department of Health, Health Standards Section, and Louisiana licenses the organization rather than the clinician. Louisiana is unusually specific about who sits outside it: the exemption list at LAC 48:I.5601(D) names an individual licensed APRN or a group practice of licensed APRNs, along with individual licensed mental health professionals, physicians and physician assistants practicing under their own license. It starts to matter when a practice moves toward a clinic or program model, adds substance use treatment, or runs intensive outpatient or residential services. The rule was amended in January 2026, so old guides are a bad source. We confirm where you land during onboarding, before it can hold up a payer contract. We don’t give legal advice and we don’t file it for you.

We handle credentialing and payer enrollment in Louisiana and in the other 49. Physicians and outpatient medical groups, psychiatrists included, start on the physician credentialing page.

Published price and scope. No call needed.

One Onboarding Packet

Short and guided. It asks more only where your answers call for it, and the hospital affiliation question is near the front for a reason.

We Prepare Everything

Portal enrollment, the Healthy Louisiana plan applications and every commercial file, built from what you told us.

You Review And Sign

Read it, change anything, sign. Nothing goes out until you say so.

We Submit And Track

Every purchase includes Command Suite, so you can see where each payer application stands, what we’re working on, what’s waiting on a payer, and what we still need from you.

What We Stand Behind

Payer decisions, timing and rates aren’t ours to promise, and we don’t promise them. Our own work is a different matter. If something we delivered doesn’t work the way we said it would, tell us within 30 days of delivery and we fix it at no cost. If we can’t fix it, or can’t deliver what you bought, we refund the part it affected. Payments aren’t refundable unless the problem is our mistake. A new request, another payer or provider, a change of scope or a new build gets quoted separately.

Everything Stays In Your Name

Your NPPES record, CAQH/DataSpring profile, Louisiana Medicaid portal account and payer contracts stay in your name, and the paperwork is yours too. You keep all of it if we stop working together.

Louisiana Credentialing Questions

Questions We Get

Can I Skip Medicaid Plan Credentialing In Louisiana If I Have Hospital Privileges?
Often yes. Under La. R.S. 46:460.61(F), added by Act 143 of 2022, a provider who holds privileges at a licensed hospital, or sits on the medical staff of a licensed rural health clinic or an FQHC, is treated as having satisfied Medicaid managed care credentialing. The plans have a defined way to apply it. AmeriHealth Caritas Louisiana, for example, asks for an email with Add ACT 143 in the subject line along with its provider enrollment form. Two limits. It doesn’t skip your state portal enrollment, and it isn’t the contract. It can still take months off a Louisiana Medicaid build, which is why we ask about it in the first ten minutes.
What Happens If A Healthy Louisiana Plan Misses The 60-Day Credentialing Deadline?
La. R.S. 46:460.61 gives a Medicaid managed care organization 60 days, with a 30-day notice on application defects and a 45-day notice if a verification hasn’t come back. If the plan fails inside those 60 days to either load you into both its claims system and its electronic provider directory or deny you, you’re considered credentialed and paid at the Medicaid fee schedule. No demand letter, no regulator. Most state remedies need somebody to complain first. This one doesn’t. But if the plan later completes the process and decides you don’t meet its requirements, it can recover the difference between out-of-network and in-network payments, but only if it told you about the adverse determination and started the recovery within 30 days of it. The dates are the whole case, so we keep them.
Does Louisiana Blue Backdate My Network Effective Date?
No. Louisiana Blue states plainly that it doesn’t backdate network participation. Your effective date is the date on the signature page of your provider agreement. You’re out of network until the Welcome to the Network letter arrives, and claims filed before that date process as out of network, which can send the payment to the patient instead of to you. Plan your go-live off the countersigned agreement, not off the credentialing approval email. Louisiana also has no equivalent of the retroactive-reimbursement rules a few other states wrote, so there’s nothing to recover afterward. It’s the most expensive assumption to get wrong in this state.
Do I Have To Enroll With Louisiana Medicaid If I Only See Commercial Patients?
If you write prescriptions for anyone covered by Louisiana Medicaid, treat it as yes. Since July 1, 2023 a pharmacy point-of-sale claim denies when the prescriber isn’t enrolled with Louisiana Medicaid. A commercial-only or cash-pay practice can still have a patient turned away at the counter because of an enrollment the prescriber never thought applied to them. The fix takes longer than the problem does, so we file it early.
How Many Healthy Louisiana Plans Are There Now?
Five, and all five are statewide: Aetna Better Health of Louisiana, AmeriHealth Caritas Louisiana, Healthy Blue, Humana Healthy Horizons in Louisiana and Louisiana Healthcare Connections. UnitedHealthcare Community Plan is no longer one of them. The Department of Health closed out that contract on March 31, 2026 and reassigned nearly 280,000 members to the other five. Some state provider pages were slow to drop the old listing, so a Healthy Louisiana contact sheet that still names UnitedHealthcare is stale. Magellan Health of Louisiana sits alongside the five under a separate contract for the Coordinated System of Care. One change is already set: Healthy Blue leaves Healthy Louisiana when its contract ends on December 31, 2026, and its members move to the remaining four plans on January 1, 2027.
Where Does A Behavioral Health Application Go At Each Louisiana Carrier?
Four carriers, four different doors. UnitedHealthcare routes behavioral health through Optum, starting on the Louisiana page and then Join Our Network inside Provider Express. Cigna routes it through Evernorth. Aetna keeps it in house, on the same request for participation as medical. Louisiana Blue brought behavioral health authorization and case management in house on January 1, 2026, and Lucet no longer runs it, so any Louisiana instruction pointing behavioral health at Lucet is out of date. Applying at the medical door when the carrier runs a behavioral one is the classic wasted month here.
I’m Opening A Second Louisiana Office. Do I Have To Credential Again?
Usually not. Under La. R.S. 22:1009(B)(5) and (6), a provider already credentialed and current at one Louisiana location is considered credentialed at any other Louisiana location where they may legally practice, as long as another currently credentialed provider legally practiced there at least 30 days earlier and you give the issuer written notice of the new location. Credentialing at that additional location is effective immediately when the issuer receives the notice, and the statute says no other requirements may be placed on you for it. Check it before anyone starts a fresh application, and keep the written notice.
Do You Need My CAQH Password?
No. We’re added as an authorized practice manager, which lets us keep your practice data current during your project and authorize payers. You complete and attest your own profile, and you sign anything that needs your signature. If you want us to maintain the profile, you share access securely and change the password when we’re done. CAQH does double duty in Louisiana, because both credentialing statutes name either the Louisiana Standardized Credentialing Application Form or the CAQH format, and Louisiana Blue, Healthy Blue, AmeriHealth Caritas Louisiana, Aetna and Evernorth all pull from CAQH. One unattested profile stalls several applications at once.
