PMHNP Practice Requirements In Iowa: Scope, Prescribing & Collaboration
Short answer: AANP lists Iowa as full practice, so a PMHNP practices and prescribes under the Iowa Board of Nursing alone, with no collaborative or supervisory agreement and no supervised hours period before independent practice (481 IAC 621.4). An ARNP may prescribe Schedule II through V controlled substances with a DEA registration and an Iowa CSA registration (481 IAC 621.6), and must register with the Iowa PMP (Iowa Code 124.551A).
Iowa PMHNP Rules At A Glance
| Topic | Iowa Rule |
|---|---|
| Practice authority | Full practice. An ARNP may assess health status, order tests, make a differential diagnosis, and develop the treatment plan to the full extent of license and education, under the Board of Nursing alone (481 IAC 621.4(4)). |
| Physician agreement | None required. The Board’s ARNP Role and Scope page says an ARNP may practice independently in Iowa and that a collaborative agreement with a physician is optional, not a Board requirement. |
| Physician limit | Not applicable. Iowa requires no physician agreement, so there’s no ratio to track, and the ARNP rules set no transition or supervised hours period before you practice on your own (481 IAC chapter 621). |
| Controlled substances | Schedule II through V allowed within the PMHNP role and population focus. You need an active DEA registration and an Iowa CSA registration from the Board of Pharmacy (481 IAC 621.4(3), 621.6(1)). |
| PDMP | Register with the Iowa PMP when you get or renew your CSA registration. A query is mandatory before prescribing an opioid, not before a stimulant or benzodiazepine (Iowa Code 124.551A; 481 IAC 621.7). |
| Regulator | Iowa Board of Nursing, now housed in the Department of Inspections, Appeals, and Licensing (dial.iowa.gov). The ARNP rules moved from 655 IAC chapter 7 to 481 IAC chapter 621 in June 2025. |

Practice Authority
Iowa Practice-Authority Category (Full Practice)
AANP currently lists Iowa as a full practice state on its State Practice Environment map. In practice, PMHNPs there evaluate and diagnose patients, order and interpret tests, and start and manage treatment, including prescribing, under the exclusive licensure authority of the Iowa Board of Nursing. The state doesn’t require a career-long collaborative or supervisory agreement.
The AANP State Practice Environment map shows how Iowa compares nationally, and AANP’s practice information by state adds state-specific detail. The rules you’re held to come from the Iowa Board of Nursing.
Collaboration
Is A Collaborative Or Supervisory Agreement Required?
In Iowa, there’s no ongoing, career-long collaborative or supervisory agreement to maintain. Look at the start of your career, though. Some full practice states require a defined transition-to-practice period, supervised or collaborative, before an NP practices fully independently, and state rule sets whether one applies to you, how long it runs and how it’s documented.

Prescriptive Authority
Prescriptive Authority And Controlled Substances
Prescriptive authority in Iowa comes once you meet the state’s requirements, and it can cover controlled substances when the conditions are met. You’ll also need a separate federal DEA registration for controlled substances, and some carry additional federal or state conditions. The Iowa Board of Nursing and state law set the scope, any limits and the documentation, and the U.S. Drug Enforcement Administration covers the federal side.

Credentialing Is A Separate Track
Scope of practice is only half of it. Getting paid by insurance in Iowa means credentialing, and that track has its own payer rules, Medicaid program and enrollment timelines. Our Iowa credentialing and payer enrollment page walks through it, and the PMHNP credentialing guide covers the national side.
Planning to open on your own? Our guide on how to start a PMHNP practice and the 90-day launch checklist lay out licensure, collaboration and enrollment step by step. Our two cost guides spell out what PMHNP credentialing costs and what it costs to start a PMHNP practice.
Already credentialed and moving to your own Tax ID? Payer Transition moves five commercial payer contracts you already hold for $2,250 per provider. Starting from zero, First Practice Launch sets up the payers, EHR, portals and billing together for $6,000, or four monthly payments of $1,500.
FAQ
Frequently Asked Questions
Can A PMHNP Practice Independently In Iowa?
Yes. Iowa is a full practice state, and 481 IAC 621.4(4) gives an ARNP authority to practice to the full extent of license, education, and experience under the Board of Nursing alone. The Board’s own guidance says a collaborative agreement with a physician is optional. There’s no supervised hours period before you can practice on your own.
Do I Need A Collaborating Physician In Iowa?
No. The Iowa Board of Nursing doesn’t require a collaborative or supervisory agreement for ARNP practice or prescribing. Employers, hospitals, and some payers can still ask for one in their own contracts, so read those documents before you sign.
Can A PMHNP Prescribe Adderall In Iowa?
Yes. An Iowa ARNP may prescribe controlled substances in Schedules II through V within the PMHNP role, which covers stimulants like Adderall (481 IAC 621.4(3)). You need a DEA registration plus an Iowa CSA registration from the Board of Pharmacy, and you must be registered with the Iowa PMP. The mandatory PMP check applies to opioids, so checking before a stimulant is good practice rather than a rule (481 IAC 621.7).
This is general information, not legal advice. It applies the general nurse practitioner rules in Iowa to PMHNPs, and those rules aren’t the same for every role. Rules change, so check with the Iowa Board of Nursing and the AANP State Practice Environment map before you act on it. Last reviewed: September 26, 2026.
