Starting A Therapy Practice In Pennsylvania (2026): Licenses, Business Setup And Insurance
Pennsylvania allows associate counselors and family therapists to receive insurance reimbursement while practicing under supervision, subject to each insurer's requirements. This guide explains licensing, Medical Assistance enrollment and commercial payer options. We prepare, submit and follow commercial payer applications until each payer gives its written decision.
Starting A Practice
Pennsylvania is the one state in this series where the Department of State says in its own words that associate counselors and family therapists can receive insurance reimbursement while they practice under supervision.
Which Licenses Can Practice On Their Own In Pennsylvania
The Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors regulates licensed social workers, licensed clinical social workers, licensed bachelor social workers, licensed marriage and family therapists, licensed associate marriage and family therapists, licensed professional counselors and licensed associate professional counselors. Psychologists are regulated by the separate State Board of Psychology.
The boards license you. Each insurance company decides whether to credential you.
Associate And Intern Licenses In Pennsylvania
Pennsylvania has an associate license that the other four states in this series do not have in the same form. The Licensed Associate Professional Counselor (LAPC) is described on the Department of State’s snapshot page as someone gaining supervised clinical experience to become a licensed professional counselor, working under the supervision of an approved licensed supervisor. Applicants submit a supervision plan and the supervisor’s credentials.
The Department’s April 15, 2024 announcement of the law says associate licensees will be able to provide mental health support and receive insurance reimbursement while practicing under the supervision of a licensed supervisor. Once the required supervised hours are done, they become eligible for the full license and independent practice. So the state itself speaks to reimbursement for associates. Ask each insurance company whether it credentials associate licensees before you plan around it.
Forming Your Business In Pennsylvania
The Pennsylvania Department of State lists a $125 fee for a domestic limited liability company Certificate of Organization.

Whether your license calls for a professional structure or lets you use a standard LLC is a legal question. We couldn’t confirm it from a state board page, and we don’t give legal advice, so ask a Pennsylvania attorney before you file.
Pennsylvania Medicaid
Pennsylvania Medicaid provider enrollment runs through the Department of Human Services and its PROMISe portal. The DHS page says practitioners in Pennsylvania must be licensed and currently registered by the appropriate state agency to be eligible, and that you use the Online Provider Enrollment portal for a new application, revalidation or reactivation. It also says enrollment in state Medicaid does not put you in any individual managed care network, and that new providers should contact each plan directly because some networks may be closed.

Medicare For LMFTs And Counselors
Since January 1, 2024, Medicare has paid marriage and family therapists and mental health counselors directly. CMS says that effective that date, MFTs and MHCs can bill Medicare independently for services furnished for the diagnosis and treatment of mental illnesses. CMS’s enrollment FAQ says to enroll through PECOS, the online system, or the paper CMS 855I application, and describes the requirements: a master’s or doctor’s degree that qualifies you for licensure, at least 2 years or 3,000 hours of post master’s degree clinical supervised experience, and a license or certification from the state where you practice.
We walk through the steps in our Medicare enrollment guide for counselors and MFTs.
