Starting A Therapy Practice In Oklahoma (2026): Licenses, Business Setup And Insurance

Opening an insurance based therapy practice in Oklahoma means knowing which licenses practice on their own, what associates can do under supervision, and how SoonerCare and the commercial insurers fit. We prepare, submit and follow commercial payer applications until each payer gives its written decision.

Starting A Practice

Oklahoma splits therapist licensing across three boards, and its counselor and family therapist rules put associates in a candidate status under supervision. The state also publishes the cost to file a professional limited liability company.

Which Licenses Can Practice On Their Own In Oklahoma

The Oklahoma State Board of Behavioral Health Licensure governs Licensed Professional Counselors (LPC), Licensed Marital and Family Therapists (LMFT) and Licensed Behavioral Practitioners (LBP). Clinical social workers are licensed by the Oklahoma State Board of Licensed Social Workers, and psychologists by the Oklahoma State Board of Examiners of Psychologists.

Those are the licensing boards. Whether an insurance company will credential you depends on the insurer, so a license opens the door and each payer decides the rest.

Associate And Intern Licenses In Oklahoma

Oklahoma calls the pre license stage a candidate. The LMFT rules define a candidate as a person whose application for licensure has been accepted and who is under supervision for licensure. The Board’s application page says what you are applying for at that stage is permission to begin your supervised practice hours, not the full license.

The LPC and LMFT rules both say that when supervised hours are earned in a private for profit or not for profit therapy setting, the candidate has to be an employee of the same facility as a licensed person whose assigned job duties include being immediately available for supervision whenever the candidate is practicing. That puts a candidate inside someone else’s practice. It is not a path to opening your own.

Forming Your Business In Oklahoma

The Oklahoma Secretary of State lists the filing fee for Articles of Organization for an Oklahoma LLC at $100.

Oklahoma also has a separate professional limited liability company form, issued under Title 18, Section 801 of the Oklahoma Statutes. Its instructions say the filing fee is $100 and that you attach a certificate from the regulating board that each owner or manager who will practice is duly licensed. Whether your license calls for the professional form is a question for an Oklahoma attorney, and we don’t give legal advice.

Oklahoma Medicaid

SoonerCare is Oklahoma’s Medicaid program, run by the Oklahoma Health Care Authority. New provider contracts are filed through the OHCA provider enrollment page, which links to the new contracts portal at ohcaprovider.com and to a separate portal for renewals. OHCA’s site also carries a SoonerSelect section for the managed care side.

Treat the state contract and each plan you join as separate steps, and read the OHCA page before you start.

Medicare For LMFTs And Counselors

Since January 1, 2024, Medicare has paid marriage and family therapists and mental health counselors directly. CMS says that effective that date, MFTs and MHCs can bill Medicare independently for services furnished for the diagnosis and treatment of mental illnesses. CMS’s enrollment FAQ says to enroll through PECOS, the online system, or the paper CMS 855I application, and describes the requirements: a master’s or doctor’s degree that qualifies you for licensure, at least 2 years or 3,000 hours of post master’s degree clinical supervised experience, and a license or certification from the state where you practice.

We walk through the steps in our Medicare enrollment guide for counselors and MFTs.
